Book of Remembrance: If you wish to have a name added to the Book of Remembrance, please fill out the application and mail it, along with a cheque for $25, to the ACW Diocesan Treasurer, Valorie Harrison: 13511-61 St., NW, Edmonton AB, T5A 0T6. Make cheque payable to: ACW Diocese of Edmonton.

Life Membership: To apply for a Life Membership, please fill out the application, and mail it, along with a cheque for $50 ($25 to be given to recipient's charity of choice), to the ACW Diocesan Treasurer, Valorie Harrison: 13511-61 St., NW, Edmonton AB, T5A 0T6. Make cheque payable to: ACW Diocese of Edmonton. All applications are subject to ACW Diocesan Board approval. Applications for ACW Life Memberships and Pins, must be submitted three months prior to the AGM or regional meetings.

Application Deadlines:

• Diocesan Annual General Meeting: last day of January

• Cold Lake Regional: last day of February

• Edmonton West/Yellowhead or Whitemud Regional: last day of May

• Battle River Regional: last day of December
